REQUIRED FORMS
The two REQUIRED forms for all wilderness medicine courses (NOT required for CPR/AED or Basic First Aid classes) are our Basic Health Form and Student Agreement. All students enrolled in a CWS wilderness medicine course must complete a Student Agreement and a Basic Health Form before they can participate in the class. Failure to complete these forms prior to your course will prevent you from attending class. These forms are located on the Course Downloads Page of your course. The password required to access your course downloads page will be provided to you upon registering for your wilderness medicine course, and is included in your enrollment confirmation email.
